Sharing a provider report

Educational guide — not medical advice. The report organizes your own records for a conversation with your provider — it draws no conclusions and makes no recommendations.

Why this exists

Appointment conversations usually run on memory: "I think I missed a couple of weeks… the nausea was maybe around the dose change?" Your StaQ record already knows the real answers. The provider report turns it into something a clinician can scan in a minute — a useful record, not a data dump.

Generating one

  1. Open the Provider tab.
  2. Pick a lookback range — for example, everything since your last visit, or the last year.
  3. Add patient notes for provider if there's context you want up front: what you want to discuss, changes you've noticed, questions you have.
  4. Tap Generate, then Share with Provider — bring it on your phone or send it ahead of the visit.

The report covers your regimen and its changes (including titrations), your actual dose history with adherence against your target dose, and the outcome data you've logged — weight, check-ins, injection-site notes.

Raw data export

If your provider's office wants the underlying records — or you simply want your data out of the app — Raw Data Export gives you everything as plain data. Your record is yours; the export makes that literal.
